How to apply for various certificates at Fukuoka Campus

We will introduce how to apply for various certificates for current students and graduates. Please note that some procedures may require a power of attorney or a copy of your identification card, and may take several days to be issued.
If you are a current student, please complete the procedures directly at the Student Support Center counter.

  • * Those who graduated from Kyushu Teikyo Junior College, Teikyo University Fukuoka Junior College, and Teikyo University Fukuoka Medical Technology College should also apply to the Fukuoka Campus Student Support Center.

  • * Excluding Saturdays, Sundays, holidays, year-end and New Year holidays (12/29-1/3), university festival holidays, entrance exam days, and other days when the counter is closed.

For those who are graduates (completed) and wish to issue various certificates

You can apply by coming directly to the counter or by mail.
Certificates for those who have graduated (completed) from another campus cannot be issued.

  • * Certificate will be issued with your name when you were in school
  • * Issuing a certificate by mail takes approximately 7 to 10 days. Please consider the current postal situation and apply with plenty of time to spare.

1. Those who can come directly to the counter

Please be sure to bring your ID (driver's license, health insurance card, passport, or My Number card) to verify your identity. Also, please bring the fee for the certificate you wish to obtain in cash. The procedure will be explained at the counter.
If you have to have a representative apply for the certificate, please also submit the following three items:

2. Those who apply by mail

Please gather the following four items (A, B, C, D) and, if necessary, (E, F) and mail them to the address below.

A: Application form for various certificates

  • * If you are unable to download the application form, please enclose a paper copy with the required information below filled in. The form can be in any format.

  1. Your name when you were in school (If your surname has changed after graduation, enter your new surname)
  2. Birthday
  3. Department and student ID number (if unknown, please enter the date of graduation/withdrawal (withdrawal, expulsion))
  4. Delivery address
  5. Phone number (number that can be contacted during the day)
  6. Where to submit the certificate
  7. Certificate type and number
  8. Reason for application
  9. Whether or not the certificate needs to be sealed

B: Copy of identification card that can confirm your identity (driver's license, health insurance card, passport, My Number card)

  • If you enclose a copy of your health insurance card, please mask any symbols and numbers of the insured person.
  • If you enclose a copy of your My Number card, please copy only the front side of it while it is in the designated case; do not copy the back side.

C: Fixed amount money order issued by the post office for the total fee for the certificate (cash not accepted)

  • Please check the fees for issuing certificates in the "List of Various Certificate Issuance Procedures," purchase a fixed-amount postal money order for the fee at a post office, and enclose it with your document.
  • Do not write anything on the fixed amount postal money order.

D:Reply envelope

Attach a stamp to a long size 3 envelope and write the recipient's address and recipient's name. The price of stamps varies depending on the number of certificates.

  • 4 to 6 copies: 180 yen
  • 7 to 10 letters: 270 yen
  • For express delivery, please affix an additional 300 yen stamp in addition to the stamps listed above.
  • If you are applying by mail, please apply well in advance, taking into consideration postal conditions.

E: Power of attorney, copy of the representative's ID (only if an agent is applying on your behalf)

F: Prescribed form for the sending party (only if the sending party has a prescribed form)

  • * Please be sure to contact the Student Support Center before applying.

Notes

  • If there are any deficiencies in the information or enclosed materials, we will not be able to issue a certificate. Please be careful not to make any mistakes.
  • If you wish to apply for a certificate other than those listed above, please contact the Student Support Center.
  • If you apply by mail and have an English certificate, it will take approximately one week for the certificate to be issued.
